Our home care packages are customized to individual needs and may include different services. Clients usually contact a homecare service provider such as CareMinders directly or indirectly through doctor and hospital referrals to discuss their needs and determine what homecare plan is best for them.
The common goal of each care package is to enable happier, more independent living for the person receiving home based care, and to provide support and peace of mind for their families.
PERSONAL CARE and COMPANIONSHIP.
Help with everyday activities like toileting, bathing and dressing, meal preparation, and minimal household tasks to enable independence and safety.
This is non-medical care requiring nurse aide services, senior nurse aide for medication needs, assistive care or companion care.
Care is provided on an ongoing basis on a schedule that meets a Service User’s needs, up to 24 hours a day, 7 days a week, including possible live-in care.
PRIVATE DUTY NURSING CARE.
This is long-term hourly nursing care at home for adults with a chronic illness, injury, or disability.
It is Private duty nursing care that sometimes needs to be prescribed by a doctor.
Care is provided primarily in shifts, up to 24 hours a day, 7 days a week.
Palliative care can entail skilled nursing live-in.
REHABILITATION CARE.
It is short-term, therapist-directed care designed to help a Service User prevent or recover from an illness, injury, or prolonged hospital stay.
It involves intermittent rehabilitative skilled care, or visiting therapy services at home.
Care is provided through specialist visits from clinicians, dieticians or even doctors.

What is Home care?
It is nursing care or personal care and support for people who cannot fully look after themselves, at times convenient to them, and in ways they find agreeable.

Is Home care available in Kenya?
Yes it is.
- Homecare means receiving nursing care in a homely environment by a person who is usually incapacitated and cannot do things for themselves.
- (BMC Palliative Care, 2011). A case study in Maua, Meru Kenya indicated that contracted care workers / nurse assistants were not a common occurrence in palliative care in Kenya. Care was mainly given by family members and stood at 83.3%. Clinical encounters however were very significant meaning that relatives correlated well the wellbeing of their patient with a visit from a medical staff.

What are your services?
According to client information provided, our clinical team will carry out a full client assessment where we can deduce on the level of care we feel the patient requires to achieving optimal health benefits.
Our 3 levels of care are:
- a) Personal care and companionship involves: - A home care aide on 12 hr, 24 hr basis or live-in care.
- Elderly care, Dementia, Alzheimer's, Physical disability etc.- b) Private duty nursing care involves: - Supervision of care by a specialized critical care nurse practitioner such as palliative care.
- Catheter care management and replacement, colostomy bag management etc.
- Nursing services from qualified nurses (KRN), nurse practitioners and senior home care aides.
- Condition-led care such as palliative care and hip replacement etc.
- Clinical interventions such as stoma and catheter care management etc.
- Degenerative diseases care such as dementia and alhzeimers.
- Temporary and permanent disability management.
- Post-surgery care.
- Occupational health services such as speech therapy.
- Medical nutrition therapy.- c) Rehabilitation service involves: - A physio / occupational therapist may be engaged dependent on mobility issues and difficulties with activities of daily living (ADLs) respectively.
- Rehabilitation services such as post stroke care, dementia care etc.
- A medical nutritionist may be engaged dependent on medical diet intake trajectory. -
What involves nursing care?
- Daily vital signs monitoring as necessary.
- Periodic head to toe physical examination as necessary with input and output records to show.
- Daily assistance with personal care and dressing.
- Depending on his / her feeding and hydration, there will be urine output monitoring.
- Blood pressure monitoring as per patient’s condition.
- Diet as per nutritionist directive.
- Monitor bowel patterns and intervene as necessary.
- keep dry all the time.
It also involves:
• Drug administration as per the doctor’s prescription.
• Blood chemistry monitoring as per doctors instructions.
• Ensure regular walks / activity as tolerated.
• Oral care three times a day as necessary.
• Provide companionship. -
What are your charges?
Our charge rates are usually determined by the level of care required. The charges will vary based on the client’s needs. The post hospitalization care is usually a 24 hours service that is custom-made depending on the client’s needs. A care plan will usually be prepared then implemented accordingly.
Our quotes are usually for professional fees and equipment hire only (where these are clearly stated) and does not include any procedures that may be carried out at home or consumables that may be required. Clients are also required to cater for transport costs for the hospital bed hired / bought and the technician cost that will assemble the bed at home.

What are your payment terms?
Our nurse-to-home services payments are STRICTLY done fortnightly. Clients will receive an electronic invoice through their appointed email account; payable within 3-5 working days from the invoice date.
Clients will pay through bank transfer to the appointed bank company account as advised. No other payment method will be accepted unless authorised by the Director for that specific client.
Payment default will mean discontinuation of service without notice. It is in your interest to pay on time once you engage us.
Our Home Care Package.
Under this service plan, the client will be entitled to the following benefits.
- Ability to access a preferred medical team.
- We will liaise with his / her primary doctors on his / her management.
- We will carry out regular health status measurement indicators e.g. blood pressure, blood glucose levels for diabetic checks, measurements of blood coagulation for patients taking blood thinning medications, blood oxygen levels and sleep apnea.
- We will assist with laboratory investigations requested by his / her doctor(s).
- In the event of an emergency, we will organize for an ambulance to the family’s hospital of choice.
